Are you Homeless? Do you have Unstable Housing?

  • 1 picture ID, 1 other ID(Social security, Birth cert, Passport, Farmworkers ID).
  • Homeless Verification form (We can assist you in getting this).
  • Paystubs or Social security benefits or Food Stamps.
  • Bank Statements or last 3 months for All Accounts.
  • Affordable care act eligibility results or DACA letter or Farmworkers Association No Social Letter.

Do you live in Someone else’s Home?

  • 1 picture ID, 1 other ID(Social security, Birth cert, Passport, Farmworkers ID).
  • Verification of Support Form, Last 3 months of mail(shows name, address, & date) or current vehicle registration.
  • Paystubs or Social security benefits or Food Stamps.
  • Bank Statements or last 3 months for All Accounts.
  • Affordable care act eligibility results or DACA letter or Farmworkers Association No Social Letter.

Do you Rent your Home?

  • 1 picture ID, 1 other ID(Social security, Birth cert, Passport, Farmworkers ID).
  • Copy of Current lease or Rent, Last 3 months of mail(shows name, address, & date) or current vehicle registration.
  • Paystubs or Social security benefits or Food Stamps.
  • Bank Statements or last 3 months for All Accounts.
  • Affordable care act eligibility results or DACA letter or Farmworkers Association No Social Letter.

Do you Own your Home?

  • 1 picture ID, 1 other ID(Social security, Birth cert, Passport, Farmworkers ID).
  • Property tax bill or Mortgage Payments, Last 3 months of mail(shows name, address, & date) or current vehicle registration.
  • Paystubs or Social security benefits or Food Stamps.
  • Bank Statements or last 3 months for All Accounts.
  • Affordable care act eligibility results or DACA letter or Farmworkers Association No Social Letter.

You can bring these documents with a walk in or you can send them to wvhahc@risingagainstallodds.com  with the Subject line : LAST NAME_FIRST NAME DOCUMENT NAME.

⚠️ Disclaimer – Please Read Carefully

We encourage all clients to arrive at least 30 minutes early, especially if you know you’ll need to complete paperwork.

When you arrive, please sign in on the date sign-in sheet located at the front desk.

If you are a new client, you will be asked to complete the following forms:

  • Client Intake Summary Sheet
  • Client Hold Harmless & Release Form
  • HIPAA Authorization Form

Many benefits are time-sensitive, and delays in documentation can affect your eligibility.

If you are homeless, please note that your Homeless Verification Letter must be updated every 6 months.

Don’t Have All Your Documents? That’s OK!

We understand that gathering paperwork can be difficult. Don’t worry, we’re here to help. Our team can assist you in obtaining documents such as: Birth certificates, State IDs, Social Security cards, Divorce paperwork, Homeless Verification forms

Please note: Some documents may take days, weeks, or even months to receive when working with government agencies. This process may require multiple appointments, and that’s perfectly normal. We’re here to support you every step of the way.

Completing these applications takes time, effort, and patience. and we know it’s not always easy. You’re not expected to have everything figured out. Just show up, we’ll guide you from there.
